As individuals age, vascular changes such as arterial stiffening and reduced baroreceptor sensitivity contribute to the rising prevalence. The cellular pathophysiology involves increased sympathetic nervous system activity, endothelial dysfunction, and altered sodium handling by the kidneys, which together contribute to elevated systemic vascular resistance and blood pressure.

Assessment and Treatment in Advanced Practice

Advanced practice nurses (APNs) must use age-sensitive approaches when assessing hypertension in older adults. This includes monitoring for orthostatic hypotension, cognitive impairment, and medication side effects. Treatment should be individualized, starting with lifestyle interventions such as sodium restriction, weight management, and physical activity. Pharmacologic management often begins with low-dose thiazide diuretics, ACE inhibitors, or calcium channel blockers, with close monitoring to avoid hypotension and falls. Genetic factors such as polymorphisms in the renin-angiotensin-aldosterone system may also influence medication response.
