In recent decades, health promotion programs in the United States have substantially improved the health of residents. However, there is ongoing concern about health disparities, which occur most commonly in racial and ethnic  people who do not have health insurance, and those who are poor and have less than a high school education. A recent report released by the Centers for Disease Control and Prevention (2011a) emphasizes that there are considerable and persistent gaps between the healthiest and least healthy people. For example, many racial and ethnic groups have higher rates of the following conditions that are amenable to preventive interventions: obesity, diabetes, hypertension, HIV/AIDS, and cervical cancer. Another disparity in preventive services is the significantly lower rates of adult immunizations among non-Hispanic Blacks and Hispanics.

Global Health Inequities

Health disparities are not limited to the United States. Globally, low- and middle-income countries face limited access to basic healthcare services, clean water, and nutritious food. Preventable diseases like HIV/AIDS and tuberculosis remain leading causes of death. Disparities often stem from poverty, lack of infrastructure, and political instability. Global health organizations focus on universal health coverage and targeted education to reduce these inequalities.

National Challenges in the U.S.

In the U.S., health disparities continue to affect millions. Racial and ethnical  experience higher rates of chronic diseases such as hypertension and diabetes. Lack of insurance and limited access to preventive care exacerbate these issues. For instance, adult immunization rates are significantly lower in Hispanic and non-Hispanic Black populations. Health promotion programs must target these gaps using culturally appropriate strategies.

Local-Level Disparities

At the community level, disparities can be seen in neighborhoods with limited healthcare facilities, poor public transportation, and low-income schools. These communities often lack health education and preventive services.
